Personal Training Contract Pause Form

Use this form to request a temporary pause of your personal training contract and payments for reasons such as vacations, travel, medical leave, or family emergencies. Suspensions apply to future dates only and cannot be backdated. During the suspension, your contract will be on hold, and no billing will occur. Your contract will automatically resume under the same terms once the suspension period ends. Please note: Open gym memberships (monthly and annual memberships for gym use) cannot be paused or suspended.
